What Are Immediate Dentures?

An immediate replacement denture is made prior to the extraction of the natural teeth and which is inserted into the mouth immediately after the extraction of those teeth. It may involve total or partial replacement.

Advantages of Immediate Dentures:

  • The original appearance of the patient’s dentition is able to be maintained / improved, by the placement of the artificial teeth in a position similar to natural teeth, or improved by changing the position.
  • The immediate replacement dentures are provided immediately, ie at the time extraction(s). Thus, the patient does not have an edentulous at the time of extraction
  • Immediate replacement dentures provide a cover for the extraction sockets; allowing for isolation of the wound from bacteria + food, and thus providing a better environment for the wound to heal.
  • Further an added advantage to the dentist is it can act as a mode of haemorrhage control
  • With the use of immediate replacement denture, one is able to maintain the soft tissue contours of face, so as to all the facial musculature to maintain its natural position and to become loose or stretched with a period of edentulism.
  • Also with the immediate replacement dentures on is able to allow the patient to main their mental + physical well being
  • Socially and publicly the patient is will not be seen as edentulous, important for social reasons, to not cause an stigmatism
  • Aesthetics are improved, or maintained
  • Adaption to dentures is aided
    • Maintain tooth position
    • Maintain muscle balance
    • Prevent formation any abnormal mandibular movements
    • Aids chewing + mastication

Disadvantages of Immediate dentures:

  • Requires a great deal of patient cooperation. In some cases multiple visits may be necessary prior to surgery, plus after future relines + adjustments may be necessary. Further is also requires the clinical have a great deal of chair time to plan + manufacture the immediate replacement dentures.
  • As is all cases, once the natural dentition is lost the alveolar bone begins to resorb this is because compressive + tension forces are necessary to maintain bone remodeling. Once the natural dentition is lost the predominate force is compression of the soft tissues, via the dentures base leading to resorption of the alveolar bone. The Alveolar bone loss occurs rapidly leading to loss of adaptation of the fitting surface of the immediate replacement denture. The loss is much greater / faster in the mandible compared to the maxilla.
  •  May require many future relines + appointments -> leading to increased cost + time
  •  No trial denture stage is possible; this is a big disadvantage, as it is not possible to show the patient what the teeth will look like aesthetically.
  •  Gross irregularities of teeth make processing difficult, such as hard tissues factors like tori, or bony spicules
  • May have poor fit, due to the fact that the teeth are removed from the diagnostic cast and not a true impression of the missing teeth.
  • May have poor retention as a further result of this factor
